NM Wellness, Lymphatics, & Rehabilitation is operated by NM Lymphatics & Rehabilitation, LLC in Albuquerque, New Mexico.
These policies apply to the website, communications, information, resources and services of NM Wellness, Lymphatics, & Rehabilitation (“NMWLR,” “we,” “us,” or “our”). NM Lymphatics & Rehabilitation, LLC also operates Lymph + Life, an online educational platform. Although NMWLR and Lymph + Life are related and may link to or reference one another, they serve different purposes. NMWLR primarily provides individualized clinical, wellness and lymphatic care, while Lymph + Life provides broader educational resources, self-assessments, courses and related content.

Privacy Policy
NMWLR respects the privacy of individuals who visit our website, contact the practice, complete screening or inquiry forms, communicate with us, or receive care.
We collect only information reasonably related to operating the practice, responding to inquiries, communicating with current or prospective clients, providing requested services, maintaining appropriate records, and improving our website and educational resources.
We do not sell personal or health information and do not disclose personal or health information to third parties for their independent marketing purposes. Certain outside service providers may process information on our behalf as necessary to provide website, communications, forms, technology, payment or other operational services.
Depending upon how you interact with NMWLR, you may voluntarily provide information including name, email address, telephone number, contact preferences, appointment or scheduling information, health and wellness concerns, symptoms or areas of concern, medical or surgical history you choose to disclose, information concerning swelling, lymphatic health, injury, recovery or other health circumstances, information submitted through screening or inquiry forms, communications sent by email or text, information necessary to provide individualized care, and payment or transaction information when applicable.
Please provide only information reasonably necessary for the purpose of your communication.
NMWLR currently uses an online screening form, “Is Lymph Treatment Right For You?”, to allow prospective clients to voluntarily provide information about their health and wellness concerns and help NMWLR understand the nature of their inquiry.
Information submitted may include contact information as well as sensitive health and wellness information voluntarily provided by the individual.
NMWLR currently uses Google Forms, a third-party technology service, to host this screening process. When an individual submits the form, NMWLR receives an email notification and accesses the submitted response through Google’s service. Information submitted through Google Forms is therefore transmitted to and processed through Google’s systems and is also subject to Google’s applicable privacy, security and data-processing practices.
NMWLR does not sell information submitted through its screening forms and does not disclose such information to third parties for their independent marketing purposes. Limited disclosure to technology and operational service providers may occur when necessary to operate the services through which the information is collected, transmitted or maintained.
Individuals who prefer not to provide health-related information electronically may contact NMWLR to discuss other available methods of communication.
NMWLR may receive sensitive health information in connection with inquiries, screening and individualized care.
Clinical records maintained directly by NMWLR are primarily maintained as paper records in a locked office. Access is currently limited to the practitioner responsible for providing care.
NMWLR intentionally limits electronic transmission of detailed health information where practical. Nevertheless, clients and prospective clients may choose to communicate through email, SMS/text, telephone, Zoom or other electronic means. No electronic method of communication can be guaranteed to be completely private or secure. Individuals are encouraged to consider the sensitivity of the information they choose to communicate electronically.
NMWLR is a healthcare practice and takes the confidentiality of health information seriously.
HIPAA applies to health plans, healthcare clearinghouses and healthcare providers that conduct specified healthcare transactions electronically. NMWLR currently does not electronically submit insurance claims, check insurance eligibility, obtain electronic preauthorizations or conduct other HIPAA-standard insurance transactions on behalf of clients.
Accordingly, NMWLR does not represent through this website that it is a HIPAA-covered entity or that every communication or technology used by NMWLR is governed by HIPAA. This distinction does not diminish NMWLR’s commitment to maintaining the confidentiality and security of health information entrusted to the practice.
If NMWLR’s business practices change in a manner that changes its legal obligations, these policies will be reviewed and updated accordingly.
Information may be used to respond to inquiries; determine whether NMWLR may be an appropriate resource; communicate about appointments or requested services; provide individualized care; maintain clinical and business records; prepare requested superbills or receipts; process payments; provide information requested by the individual; send communications for which consent has been provided; administer and improve the website; maintain website and information security; comply with applicable law; and protect NMWLR, its clients and others from fraud, misuse or unlawful activity.
NMWLR does not sell or rent personal or health information.
Information may be disclosed when reasonably necessary to technology or operational service providers acting on our behalf; to process a service or request initiated by you; when you authorize or request the disclosure; when required by applicable law, legal process or governmental authority; to protect the rights, property or safety of NMWLR or another person; or in connection with professional, legal, accounting or technical assistance where appropriate safeguards are used.
NMWLR may communicate with current or prospective clients through email in response to inquiries, regarding appointments or services, or when the recipient has requested or consented to other communications.
Email is not guaranteed to be completely secure. Individuals choosing to communicate sensitive or health-related information through ordinary email should understand the inherent privacy limitations of electronic communication.
NMWLR may also send educational, informational or promotional email communications to individuals who have requested or consented to receive them. Commercial email communications will be sent in accordance with applicable law, including requirements concerning sender identification, truthful subject lines, a valid physical postal address and a clear means of opting out of future marketing communications.

NMWLR may communicate by SMS/text message or telephone when an individual provides a telephone number and requests, initiates or consents to that communication. Communications may include responses to inquiries, scheduling and appointment information, service-related communications, follow-up communications, and other correspondence appropriate to the individual’s relationship with NMWLR.
Message frequency may vary. Message and data rates may apply depending upon the recipient’s mobile plan. Recipients may request that text communications stop by replying STOP or UNSUBSCRIBE. Recipients may reply HELP for assistance or contact NMWLR directly.
Consent to receive promotional text messages, where applicable, is not intended to be a condition of purchasing services.
NMWLR may use Zoom or another video platform for consultations, individualized education or other remote interactions. Participation in a video session necessarily involves transmission of audio, video and information through the technology provider’s systems.
Individuals should participate from a location in which they are comfortable discussing personal information and understand that internet-based communications cannot be guaranteed to be completely private or uninterrupted.
NMWLR.com and the technologies used to operate the website may automatically process limited technical or usage information necessary for website functionality, security, performance and administration.
The website may use cookies or similar technologies for ordinary WordPress functionality, preferences, security or other operational purposes. Browser settings generally allow visitors to control or refuse cookies, although disabling certain cookies may affect website functionality.
NMWLR currently does not intentionally use Google Analytics, Google Tag Manager, Meta/Facebook Pixel or similar third-party advertising or behavioral-tracking technologies to monitor visitors across websites for advertising purposes.
Certain WordPress plugins used by NMWLR may collect limited technical, aggregate or usage information relating to operation of the website or the software itself. NMWLR does not use website analytics information to sell personal information or to create advertising profiles of individual visitors.
Third-party technologies and website functionality may change over time. NMWLR may update this Privacy Policy when material changes are made to the technologies used on the website.
NMWLR may use or link to services operated by third parties, including website technology providers, Google Forms, email services, Zoom, payment providers, Lymph + Life and other educational or professional resources.

Lymph + Life is an educational platform operated by the same underlying business entity as NMWLR.
NMWLR may link to Lymph + Life resources, including educational materials, courses and self-assessment tools. Information submitted directly to Lymph + Life may be governed by the privacy and terms applicable to that platform.
Participation in Lymph + Life education does not by itself establish a clinical practitioner-client relationship with NMWLR.
The TLC Self-Assessment is an educational self-assessment associated with Lymph + Life. It is intended to help individuals consider experiences they may be noticing and learn their reported level or degree of lymphatic congestion within the framework of the assessment.
The TLC Self-Assessment is not a medical test, diagnosis or individualized clinical evaluation, and its results should not be interpreted as establishing the presence or absence of a medical condition.
When electronic payment processing is used, payment information may be processed through a third-party payment processor. NMWLR does not intentionally collect or retain complete payment-card information unless specifically necessary for an authorized transaction or business purpose. Applicable payment providers maintain their own privacy and security practices.
NMWLR uses reasonable administrative, physical and technological practices appropriate to the nature of the information maintained. However, no website, email account, text-message system, internet transmission, cloud service or other method of electronic storage or communication can be guaranteed to be completely secure.
Nothing in this Privacy Policy should be interpreted as a guarantee against unauthorized access, loss or security incidents.
